A 40-year-old male patient came to the emergency room at Hung Vuong General Hospital (Phu Tho) with severe pain in the epigastric region, abdominal distension, abdominal CT results detected acute pancreatitis, triglyceride test (medium fat). count) in the blood is high, the blood taken out is milky white. It is known that the man has a history of heavy drinking, cirrhosis, hepatitis C virus.

Immediately, the patient was assigned an emergency plasma exchange during the night. Dr. Luong Minh Tuan, deputy director of the hospital, directed the dialysis team to quickly set up the plasma exchange procedure and conduct the technique.

Plasma replacement therapy involves removing a large amount of plasma (usually 1-1.5 times the patient’s plasma volume) and then replacing it with an appropriate amount of the same volume. Blood cells separated from plasma are returned with replacement fluid to maintain intravascular volume.

Substitutes for the patient’s plasma can be 4%-5% albumin, or fresh frozen plasma (FFP). Cases of acute pancreatitis due to hypertriglyceridemia usually respond very well to this treatment.

After plasma exchange, so far, the patient has had a positive change, no more abdominal pain, blood triglycerides decreased.

Doctor Tuan said that the symptoms of acute pancreatitis were rampant such as bloating, abdominal pain, nausea, pain in the epigastrium (just below the breastbone), and bowel obstruction. Acute pancreatitis has a complicated and rapid progression. Even when the crisis is over, complications of infection, poisoning, and exhaustion are still lurking. Besides, the disease is easy to recur many times. Therefore, patients always need to be closely monitored medically and strictly follow the treatment instructions of the doctor.
