A 14-year-old male patient had to be treated for 5 months for acute pancreatitis and still could not be discharged. Initial symptoms are abdominal pain, vomiting after fatty meals.

On June 7, the City Children’s Hospital (HCMC) said it was treating a 14-year-old child TT because of acute necrotizing pancreatitis. I was admitted to the hospital with symptoms of severe vomiting and systemic toxic shock. In addition, I have a history of abdominal pain, intestinal infections many times.

The patient was transferred from Soc Trang province to Ho Chi Minh City during the Lunar New Year holiday. Doctors diagnosed her with severe necrotizing pancreatitis, pancreatic pseudocyst and had to undergo urgent surgery to remove the pancreatic cyst. Then, correct the encirclement infection thoroughly and treat it aggressively. She had a jejunal catheter inserted to give powder and crushed nutrients to support eating.

After more than 5 months of treatment, the patient is now recovering, gradually adapting to pure foods and solid foods. The doctor said that T. had acute pancreatitis right after eating greasy meals, a common condition because of acute pancreatitis in young children.

According to doctor Nguyen Cat Phuong Vu, City Children’s Hospital, acute pancreatitis in children is a big concern for parents. If not diagnosed and treated in time, the child will have dangerous complications.

The typical symptom of acute pancreatitis is sudden abdominal pain, usually after a fatty meal. Children have pain around the belly button or pain in the area above the belly button, the pain increases, especially more pain after eating. Children with acute pancreatitis may vomit a lot, be tired, have obvious signs of dehydration, eat less, have fever, chills, and have a fast heart rate.

Acute pancreatitis in children is treated by inserting a tube into the stomach to drain the fluid. Pediatric patients will have to fast, receive intravenous feeding or put a special tube through the stomach for the entire time of treatment.

After that, the patient is rehydrated, antibiotics may be required if the disease is severe. A child’s hospital stay will be very long, possibly several months.

Pancreatitis is a rare disease in children, so it is often overlooked. The disease sometimes has atypical symptoms, easy to confuse with other gastrointestinal diseases. If diagnosed and treated late, the disease progresses complicatedly, causing dehydration due to excessive vomiting, low blood pressure, abdominal distension, paralytic ileus…

Common causes of acute pancreatitis are viruses, trauma, gallstones, biliary sludge, worms in the bile ducts, and drug use (Valproic acid, azathioprine..). Less common causes are systemic, metabolic, mutational, or idiopathic.

Most pediatric patients will be stable within 7-10 days, about 13-20% will be prolonged and have complications.
